Plan payments are due by the 22nd of each month. The following payment options are available.

Mail your Payments • Your payment must be made payable to: Jack Gooding, Trustee. • The payment must reflect your CASE NUMBER and YOUR NAME as reflected on your case when it was filed. • Payments sent by regular mail MUST be mailed to our bank lockbox at: Jack Gooding, Trustee 4030 Momentum Place Chicago, IL 60689-5340 • Payments sent by “overnight delivery” using Federal Express or UPS cannot be delivered to the above address.
